Five Dimensions Investment Management LLC (“Five Dimensions”), a Florida limited liability company, was formed in November 2021. Five Dimensions commenced operations as a Family Office on February 18, 2022 to bridge the United States Securities and Exchange Commission or state securities authority registered investment adviser compliance process with maintaining continuity of the small cap strategy and performance track record. Jason Bulinski, founder and portfolio manager of Five Dimensions (the “Portfolio Manager”), is the principal owner of Five Dimensions. 

1. Five Dimensions Investment Management, LLC is defined as an independent investment management firm that is not affiliated with any parent organization. Performance shown prior to February 18, 2022 represents results achieved solely by Jason while he was part of First Midwest Bank. Policies for valuing portfolios, calculating performance, and preparing compliant presentations are available upon request. Five Dimensions Investment Management, LLC claims compliance with the Global Investment Performance Standards (GIPS®) and has prepared and presented this report in compliance with the GIPS standards. The performance has been independently verified through 31 December 2021. The verification report is available upon request. Verification assesses whether (1) the firm has complied with all the composite construction requirements of the GIPS standards on a firm-wide basis and (2) the firm’s policies and procedures are designed to calculate and present performance in compliance with the GIPS standards. Verification does not ensure the accuracy of any specific composite presentation.

2. The Small Cap Equity Composite includes all portfolios that invest in small-cap U.S. equities with the goal of providing long-term capital growth from a well-diversified strategy. The composite was created December 1, 2010. 

3. The benchmark is the Russell 2000 Equity Index. Benchmark returns do not reflect the deduction of advisory fees, custody fees, transaction costs, or other expenses of investing. Total benchmark returns assume the reinvestment of dividends and other earnings. An investor cannot invest directly in an index. 

4. Valuations are computed, and performance is reported in U.S. dollars. 

5. Gross-of-fees returns are presented before management and custodial fees but after all trading expenses.  The management fee schedule is as follows: 1.00% on the first $50 million; 0.90% on the next $25 million, 0.75% on all assets over $50 million

7. Internal dispersion is calculated using the equal-weighted standard deviation of annual gross returns of those portfolios that were included in the composite for the entire year. For those years with less than or equal to 5 portfolios, the calculation of internal dispersion is not a meaningful statistical measure. 

8. The three-year annualized standard deviation measures the variability of the composite, and the benchmark returns over the preceding 36-month period. 

9. The firm’s list of pooled fund descriptions for limited distribution pooled funds is available upon request.

10. The minimum account size for the composite was one million dollars from January 2007 through December 2015. The account minimum was changed to $500,000 beginning January 1, 2016.

11. The measure of dispersion shown is the gross annual highest and lowest returns for accounts included for the entire year. For those periods with five or fewer accounts included for the entire year, “n/a” is noted because the dispersion is not considered meaningful.

12. A large cash flow is defined as a cash flow greater than 10% of a portfolio’s market value. When these cash flows occur, performance is calculated for interim periods and geometrically linked to calculate the period returns so as not to distort performance. Prior to January 1, 2011, the modified BAI method, also referred to as the Linked IRR method, was used and accounts were valued on a monthly basis.

13. The three-year annualized standard deviation measures the variability of the composite gross returns, and the benchmark returns over the preceding 36-month period.

14. Advisory only assets are assets for which First Midwest Bank Wealth Management provided sub-advisory of its small cap strategy to Chicago based RIA Premier Asset Management investment using their chosen trading brokers and client custodians. Advisory-only assets are presented as supplemental information.
